Forms

All sports require a physical dated AFTER April 1, 2022, a concussion form, cardiac arrest form, parent permission slip, and emergency medical card on file before the athlete is allowed to practice.  Email Mike Mays at mmays@danville.k12.in.us or call his office number at 317-718-2335.

Everything is done online this year through Final Forms.  Nothing is handed into the school.
